Clubs often use 3rd party resources to set up pre-registration. Google docs, wordpress website, eventbrite. This is a great question to post on the discussion page to get suggestions. I (Jason) do registration forms for my events and sometimes for events I am running. I use formidable or woocommerce on wordpress.
